On April 1, 2022 and onward, applicants of Intellectual Disability Certificate (Ryoiku Techo) are required to identify their individual numbers (“My Number”) for the new application as well as for the renewal application by the present certificate holders. When you apply, please bring
a. Your present certificate (if you have one),
b. Your photograph (3cm width X 4cm height),
c. “My Number Card” or any other documents certifying your individual number (e.g., individual number specified resident registration certificate, etc.), and
d. Personal identification (e.g., driver’s license, passport, resident card, etc.).
※If the application is made by a third party on behalf of you, a Power of Attorney and the third party’s ID are required.
