Minoh City sent a pre-vaccination medical checklist (health condition form) for Diphtheria and Tetanus vaccination (DTV) to eligible individuals by mail in late May. The vaccination is for 6th grade elementary school children living in Minoh City. Children from 11 years old until before their 13th birthday can get the vaccination.
Please read the instructions enclosed with the checklist on how to get the vaccination. If you have not received the pre-vaccination medical checklist, please contact the office below.
