Please make use of “Minoh Magokoro Ouen Card.” This card offers services such as discounts or presents at about 500 shops in Minoh City just by showing it at each shop. It can be used in shops with a sticker. Each shop has different offers. If you want to know the shops where you can use the card or what kind of services they offer, please check “Minoh Magokoro Ouen Card” on the Minoh City website.
◆Qualification for application
Those who live in Minoh City and meet the following (1), (2) or (3) conditions.
(1) Families raising children under 18 years old (including children who turn 18 from April of the year in which you apply for the card to the end of March of the following year)
(2) Those who are 65 years old or over
(3) Those who have a Disability Certificate
◆What you need for the application
a. A certificate that can confirm the address of the applicant
b. Documents certifying the date of birth of children under 18 years old or those who are 65 years old or over (My Number card, driver’s license, Health Insurance Card, Child’s Medical Card, etc.)
c. As for a person who has a Disability Certificate, one of these Physical Disability Certificate, Medical Rehabilitation Handbook or Mental Disability Certificate
◆How to Apply
a. Bring a. and b. or c. above to one of the following places.
– Minoh City Office: Minoh Sales Office (Minoh Eigyo-shitsu), Citizen’s Registration Division (Madoguchi-ka), or General Reception of Child & Youth Support (Kodomo Sogo Madoguchi)
– General Health and Welfare Center (Sogo Hoken Fukushi Center) at Minoh Life Plaza
– Toyokawa Branch (Toyokawa Shisho)
b. You can also apply by post. Please enclose your telephone number, a note saying, “I would like to apply for a card” and a copy of the certificates above and send it to Minoh Sales Office (4-6-1, Nishishoji, Minoh 562-0003).
