Medical Care Certificates of severely-disabled people and single parent families need to be renewed every year on November 1. Minoh City has sent the renewal application forms to the eligible citizens, so please submit the form if applicable. Those who submitted the renewal applications should have received the renewed certificates in the middle of October. Upon receipt, please check your information on it.
※There are restrictions on income to be eligible for the medical care expense support. Please ask the Minoh City Office for details.
◆Severely-disabled residents who are eligible for the medical care expense support:
(1) Those who have Physical Disability Certificate Category 1 or 2 (Shintai Shogaisha Techo 1 or 2)
(2) Those who have Medical Rehabilitation Handbook Category A (Ryoiku Techo A)
(3) Those who have Medical Rehabilitation Handbook Category B1 (Ryoiku Techo B1) with the Physical Disability Certificate
(4) Those who have Mental Disability Certificate Category 1 (Seishin Shogaisha Techo 1)
(5) Those who have Designated Intractable Disease (Specific Disease) Certificate (The Shitei Nanbyo Tokutei Shikkan Jukyushasho) with the Disability Pension, and receive the Special Child Raising Allowance, Category 1
◆Single parent family who are eligible for the medical care expense support:
(1) Children being raised by a single parent (The age cut-off is in March, the end of fiscal year, for children reaching age 18.)
(2) Single parents raising the eligible children
(3) Guardians raising the eligible children
