This is for NHI (National Health Insurance) holders. If you are hospitalized or undergo surgery, your medical expenses will be expensive. By presenting the “Eligibility Certificate for Ceiling-Amount Application” (Gendogaku Tekiyo Ninteisho) to the hospital, the amount you have to pay may be reduced down to the ceiling-amount. How far it is reduced depends on your age, your previous year’s income, etc.
You can apply for this certificate whenever you need it. Once applied for, it is valid until the next July 31; after August 1 when it expires, you must apply for it again.
Those who need this certificate should apply at the Medical Insurance, Pension, & Nursing Care Counter at the City Office. When applying, you are required to bring (1) your NHI Certificate, (2) your ID card (driver’s license, Resident Card, etc.) and (3) your “My Number” identifying document (“My Number Card” or a certified copy of your residence registration with My Number on it, etc.). You can also apply by mail. You must have declared (or must declare) your income for the previous year (2022, if you apply this year).
※For Certificate holders:
Your present certificate expires on July 31, 2023. If you continue to need the certificate, please apply for a new one at the Medical Insurance, Pension, & Nursing Care Counter.
