This is a notice to those who have National Health Insurance. When you are hospitalized or undergo surgery, the amount of money you pay to the hospital (medical expenses) will be high. If you show the “Certificate of Eligibility for the Maximum Amount Applicable to Medical Expenses” at the hospital, the amount you pay may be reduced. How much cheaper will depend on your age and your income from the previous year. You can apply for this certificate whenever you need it. Once applied for, it can be used until the next July 31; after August 1, it will no longer be available, in which case you must apply again. Those who need this certificate should apply for it at the Medical Insurance, Pension, & Nursing Care Counter at the City Office. When applying, you will need (1) your National Health Insurance Certificate, (2) proof of identity (driver’s license, Resident Card, etc.), (3) something that shows your My Number (My Number card or a certified copy of your residence registration with My Number on it, etc.), and (4) your inkan (seal). You can also apply by mail.

The current certificate expired on July 31. If you still need this certificate, please apply for a new one at the Medical Insurance, Pension, & Nursing Care Counter.
