Minoh City sent a pre-vaccination medical checklist (questionnaire about physical conditions, etc.) for Diphtheria and Tetanus vaccination (DTV) to those who are eligible, by post late in May. The eligible children are 6th graders in elementary school living in the city, but other children between the ages 11 and 12 years old (before turning 13) can be vaccinated too.
Please read the instructions enclosed with the checklist on how to get the vaccination. Those who have not yet received the pre-vaccination medical checklist, please contact below.
