The above certificates, which reduce medical care expenses, were renewed as of November 1. The application forms (Koshin Shinseisho) were sent to those who are eligible and need to re-apply. The new Medical Care Certificates were sent at the end of October to those who had already applied for a renewal. If you have not applied yet, please submit the form to the counter of the Minoh City Office as soon as possible.
※The following people are eligible to receive medical subsidies from the city. Depending on your income, you may or may not be able to receive this support. Please ask Minoh City Office for details.
(1) Severely disabled people who are eligible for medical care expense support:
a. Physical Disability Certificate Category 1 or 2 (Those who have the Shintai Shogaisha Techo 1 or 2)
b. Medical Rehabilitation Handbook Category A (Those who have the Ryoiku Techo A)
c. Medical Rehabilitation Handbook Category B1 (Those who have the Ryoiku Techo B1) with the Physical Disability Certificate
d. Mental Disability Certificate Category 1 (Those who have the Seishin Shogaisha Techo 1)
e. Designated Intractable Disease (Specific Disease) Certificate (Those who have the Shiteinanbyo Tokuteishikkan Jukyushasho) with the Disability Pension or the Special Child Raising Allowance Category 1
(2) Single parent family’s medical care expense support:
a. Children being raised by a single parent (The age cut-off date is Mar. 31, the end of the fiscal year for children reaching age 18)
b. Single parents raising children by themselves
c. Guardians who are bringing up children whose parents either passed away or are unable to care for them
